Must have lived in the Commonwealth of Pennsylvania for the previous ten consecutive years

Have not been disqualified from service as a volunteer as a result of a conviction of one or more of the offenses listed under Title 18 of the Pennsylvania Crime Code or equivalent crime under federal law or law of another state, or the attempt, solicitation or conspiracy to commit any of the offenses.

Volunteer must sign affidavit and have it notarized.
